- PLEASE NOTE:
- These forms are only to be used when your team is traveling to an out-of-state tournament or if your team is using Guest Players for an out-of-state tournament.
- If you intend to have Guest Players you must FIRST submit either "Form 1: Notification of Travel to a US Youth Soccer Event" or "Form 2: Notification of Participation in Another Organization's Event" before completing "Form 3: Verification of Guest Player Form"
Three different authorization forms are available from the drop-down menu below:
Form 1: Notification
of Travel to a US Youth Soccer Event
Form 2: Notification of Participation in Another Organization's Event
Form 3: Verification of Guest Player
To obtain
permission to travel to any event affiliated with the US Youth
Soccer (USYSA) or US Soccer (USSF), the coach or manager of an Iowa Soccer Association-registered team need only complete Form 1, and Form 3 if you have guest player(s). After
the user clicks "Submit", the system generates an
approval form that includes Iowa Soccer Association's official
mark. This form should be printed out and signed by the team
official to accept the terms of the form and then presented
to the tournament or event organizer. No fees will be charged
for e-Travel forms.
Form 2 should be used for those events that DO
NOT have US Youth Soccer permission to host. This includes
all US Club events.
Your team coach or manager is responsible to know if the event is USYSA or USSF affiliated or not.
