The purpose of the Iowa State Cup is to identify the team in each age division that will represent the State of Iowa at the US Youth Soccer National Championship Series Midwest Region Championships. The Iowa State Cup is considered the highest level of youth competition in the state for Level 1 teams. |

U17G Meggie Malm Cup
Meggie Malm was a talented Iowa youth soccer player who continued her career at the University of Missouri, not only as an accomplished player, but as an outstanding student. In honor and memory of Meggie, we recognize her through the naming of an Iowa State Cup division: The Under 17 Girls Meggie Malm Cup. Click on her photo to read more about Meggie.

| The following constitutes "Final Paperwork". For due date see column corresponding to your respective State Cup age group. Paperwork is physically due in the office by 4pm on the date of the deadline. The completed paperwork may be emailed to me (jwood@iowasoccer.org), or you can drop them in regular mail, or you can hand deliver to Office. |
FINAL PAPERWORK DOCUMENTS (all documents can be completed on your computer, saved, and emailed to Jenny)
► National Championships Roster (typeable excel file)
► Verification of Eligibility Form (typeable word file)
► Copy of your "Iowa Soccer-approved" Roster (available as a .pdf from your club registrar, Sample Iowa Soccer-approved roster)
ALLIANCE 4 ALL COMMITMENT
► I'm constructing a way to capture this commitment electronically so that you do not need to collect signatures.
► Each player, coach and at least one parent will need to complete viewing of their respective under-10 minute Alliance 4 All module and then complete the online commitment form.
► Details made available week of September 19.
► Deadline for each participant to complete will be October 17.

| The only person required at team check-in is the team coordinator, or if none, the team coach. Typically, Des Moines area teams check-in on the Wednesday (for fall '11 it is THURSDAY) prior to the first weekend of play. Out of town teams check-in at tourney admin. tent no later than one hour before their first match. Jenny will confirm local check-in date/time with coordinators. |
You will present the following four things at team check-in, in alphabetical order: |
| 1) The number of copies of your National Championship Roster that corresponds to the total number of games your team could play; after presenting, these go to the team coach who uses it as your Official Game Roster and turns in one to referees before each match. After the match, the referees turn it in to me at the administrative tent. |
| 2) Acceptable proof of age, as per the rules |
| 3) Player and coach passes with current photo and sealed closed (NOTE: coach passes must correspond with the team; player passes do not need to). These are white, not to be confused with the color passes you likely use for IPSL. Also note that the "hard coach pass" is not accepted at State Cup, due to National rules. A coach must have a white pass that corresponds to each team he/she will coach at State Cup. |
