Three different authorization forms are available:

(1) Notification of Travel to a US Youth Soccer Event
(2) Notification of Participation in Another Organizations Event
(3) Verification of Guest Player

To obtain permission to travel to any event affiliated with the US Youth Soccer, USSF, the coach or manager of an Iowa Soccer Association registered team need only complete these forms online. After the user clicks "Submit", the system generates an approval form that includes Iowa Soccer Association's official mark. This form should be printed out and signed by the team official to accept the terms of the form and then presented to the tournament or event organizer. No fees will be charged for e-Travel forms.

Before you choose a form, be sure you know who has sanctioned the tournament you plan to attend. The Notification of Participation in another organizations event should be used for those events that DO NOT have a US Youth Soccer permission to host. This includes all US Club events.
