Final Paperwork
Ugh, paperwork!
Final Paperwork is physically due in my office by 4pm on the date of the deadline. The completed paperwork may be emailed to me (
jwood@iowasoccer.org), snail mailed to me, or you can hand deliver to the office.
- Please make it easy on yourself and email! Once received, I'll shoot you a message that I have it.
- The Monday after the deadline passes I will go through all of it and if you're missing anything, I'll get back to you.
Forms you'll need
You can download these forms, save on your computer, fill out, re-save and email back to me as attachments, or mail/deliver completed forms to me.
- National Championships Roster (typeable excel file)
- Before you send me this form, make sure you make as many copies of the completed form as matches your team will/could play in State Cup.
- Verification of Eligibility Form (typeable word file)
- A copy of your "Iowa Soccer-approved" Roster, available as a .pdf from your club registrar
