Region II Championships Details for Iowa State Cup Champions and our Wildcard Draw(s)

Team coordinators and coaches, as with State Cup there are many things that need to be taken care of prior to arriving at the Regional Championships. Please carefully read below and on the Region II Championships webpage (link below) and then contact me if you have any questions. . Below are some very important details to begin preparing for, as there are deadlines involved. Other things you will find on the Region II Championships webpage include rules (important!), schedules, information about the athletic trainers, field maps, etc...so please make sure you direct parents to the Region II Championships webpage as well.
REQUIRED, TEAM COORDINATORS
Hotel reservations at the designated properties. You are required to stay at the designated properties! As per the rules of the regional event, all teams are scheduled to arrive Friday, June 20 and there is a three-night minimum stay required for all reservations. Once you have reached the June 3 cutoff time for reservations, they are non-refundable and may not be cancelled. All credit cards will be charged an advance deposit on June 5th for the three-night minimum.

REQUIRED, COACHES/ANY BENCH PERSONNEL: Concussion Education Protocol
As per the law in the state of Ohio, ALL coaches or other personnel who intend to be on the sideline with a team must complete the concussion education protocol; proof of completion will have to be turned in at team check-in June 20. Coaches, when you complete, make sure you get a copy of your certificate of compliance to your team coordinator for check-in.
Each state association participating in the 2014 US Youth Soccer Midwest Regional Championships are responsible to monitor and track their coaches’ compliance with the concussion training requirements of the Return to Play law. Each coach must receive concussion training prior to participation in the 2014 US Youth Soccer Midwest Regional Championships (See O.R.C. 3707.511 (C)(1).)
Team coordinators must collect and maintain certificates of compliance from each of their coaches and will be asked for evidence of compliance at registration. Coordinators must evidence of compliance for each coach and member of bench personnel at check-in June 20.
Coaches/bench personnel may choose training from either the Center for Disease Control (CDC) or the National Federation of State High School Associations (NFHS). Both programs are free of charge. The NFHS requires a user to login but allows for tracking of progress and allows users to log in and print their certification should they require a new certificate.
Remember, upon completion, get your certificate/evidence of completion to your team coordinator! The Region has made following Ohio law a point of emphasis and exceptions will not be made.
Required: patches worn on uniforms. Jenny will distribute patches and directions to U13-U14 coordinators this weekend at Cownie after the Finals matches and will mail patches to U15-U19 coordinators the week of June 2.
Pre-paid parking passes: if you don't buy ahead of time, you can wait in line and buy them at the fields
Pre-sale merchandise: merchandise will be sold onsite, but the option to buy ahead of time is available to your teams

On the Region II Championships webpage, the "Team Information Letter" outlines that which you need for check in Friday, June 20 at 4:20pm. As with State Cup, only Team Coordinators are needed at check-in. It is held at the Dayton Marriott, 1414 S. Patterson Blvd. Note that check-in times are assigned by the Region and this year, Iowa is assigned the 4:20 timeslot.

- Regionals Roster that is generated from the electronic submission you will be making via the American Eagle online tool. Once approved by Nancy and frozen by the Region, this version will be available for you to print. Bring multiple copies, just in case!
- The three forms referenced above: Team Eligibility, Team Information, Code of Conduct
- Laminated player and coach passes. Coach passes MUST correspond to the team!
- Birth certificates/approved proof of age document for each player
- Medical release forms
- Please note that the "Team Informational Letter" on the Region II Championships webpage states these must be notarized. I have verified with the Championships Director that while preferred, there is NO REQUIREMENT for them to be notarized.
- Coach Concussion Education Certification Forms: one for each coach/bench personnel who intends to be on the sideline with your team!
